Complete the 2010-2011 National School-Based Health Care Census! 

Once again it is time for the triennial nationwide census of school-based, linked, and mobile health care programs.

The census effort is of critical importance to assessing the growth of the field, describing the diversity of the model, and advocating to national, state, and local policy makers. 

Complete the Census Online:

STEP 1: Enter the following link into your browser: https://survey56.spss-asp.com/survey?I.Project=KWAR8ESVJMA

STEP 2: Indicate it is your first time entering the online survey for the 2010-2011Census. Please complete all fields in the registration page. You can log back in using your email address.  

STEP 3: Select SBHC(s) for which you will complete the survey. Also, select Add SBHC if any of your centers are not listed.

STEP 4: Confirm that the contact information for the SBHC is accurate in the database.

You may also print a paper version of the survey by downloading it here. NASBHC recommends that the paper survey is used for preparation and reference while filling out the online survey.
